Slack Feedback Bot

A Slack bot that allows team members to submit private feedback about teammates. Feedback is stored securely in Google Sheets and only visible to leadership.

Why do we have this? We need your help! As we keep growing, it’s important to flag feedback issues before it’s too late.

How It Works

Submitting Feedback

  1. Type /give-feedback in any Slack channel or DM

  2. A private modal will open with the following fields:

Category of feedback (see options below)

Severity / Impact

How significant is this feedback

Feedback details

Describe what happened, when, and the impact

  1. Click Submit when done

Feedback Types

  • Performance / quality of work

  • Communication / responsiveness

  • Ownership / responsibility

  • Behavior in meetings

  • Other

Severity Options

  • Positive - important: Noteworthy positive behavior to recognize

  • Area of improvement - important: Significant concern that needs attention

  • Minor note / observation: Small observation for the record

What Happens After Submitting

  1. You receive a confirmation via DM with a summary of your submission

  2. Leadership is notified automatically via DM with the full feedback details

  3. Feedback is stored in a private Google Sheet for tracking and review

Privacy

  • Feedback submissions are private — only you and leadership can see them

  • The person you're giving feedback about is not notified

  • All feedback is logged with your name for accountability

For Leadership

Leadership members have access to an additional command:

  • /feedback-report — View and filter all submitted feedback

This command allows filtering by team member, date range, feedback type, and severity.
